Northwest Anesthesia, P.A. is a professional corporation, independent
from the surgeons and the hospital. The professional fee for our
anesthesia service is separate from those of the surgeon's charges,
any affiliated professional services provided, or the facility where
the service was rendered. You will receive a separate, itemized
bill from the hospital that includes "anesthesia charges". This
itemizes the usage of anesthesia equipment, supplies and drugs.

Our statements will often include charges for both the MD and
the CRNA. Our practice provides services in the "team mode", with
the medically directing MD working in concert with our CRNA to provide
the most comprehensive delivery of care to you. When this occurs,
the total bill is roughly "halved" to reflect the contributions
of both providers. Minnesota statutes recognize the practice of
advanced practitioners, including CRNA's, allowing such providers
to receive direct payment for services provided. There will be instances
where the MD or CRNA are performing anesthesia services independently
as the situation warrants, and you will receive a statement reflecting
only those services.

Northwest Anesthesia attempts to contract with as many insurance
companies as possible. Please understand that we cannot contract
with all agencies that agree to cover both the MD and CRNA charges.
If out-of-state companies seek to provide coverage to insureds,
they need to abide by Minnesota law. Minnesota insurance companies
are required to provide payment for both providers. Inquiries or
